It happens whether it's an originating thread or a response to another thread. I hit Submit and an error comes up that says "database error". I then hit my browsers back button and submit it again and it tells me that it's stopping me from making a duplicate post.

I've had that happen occasionally, and it's annoying! Is this literally happening every time you post? Have you tried all the usual stuff--clearing cache and cookies, trying a different browser, blah blah blah?
If it continues, you might want to post on the KFN feedback board. -
Known issue, and unfortunately quite common atm
Seems for me it is 100% when editing a post (which I do a lot)
Just ignore the error and reload the page, you should see any edits, new reply et al
.Don't go back a page or anything, that will definitely not help
Just reload the current page and see if things are good there before typing out something all over again
