Wont be helpful, but my gut feeling is that you have a failing webcam; likely an intermittent contact failure. This is a laptop? Make a and model? How old?

This model seems to have a lot of webcam issues reported on Lenovo's forums.

          For example
          https://forums.lenovo.com/t5/Gaming-...age=1#10111136

It may be worth checking the Vantage app in Windows; it may be mucking with hardware settings.
If the laptop has an F-key combo (such as fn-F3 or something) that toggles the webcam, try toggling it.
Also check if Windows Fast Startup is enabled - this can affect hardware in other operating systems.

But it being intermittent seems to indicate a loose connection internally as a likely cause.
